Our mission is to provide leading dementia services that enable and empower all people affected with dementia. We believe in co-caring.
Our staff are located in offices throughout greater Auckalnd and are available to make home visits when required in the Rodney District, North Shore, Waitakere, Auckland and Counties Manukau. In collaboration with other agencies we provide support to all cultures. Dementia Keyworkers offer a comprehensive range of help and support in the form of phone advice and consultations, Home visits, Carer support groups, activity groups and carer education programmes.
Our members include those who have these conditions, their family/whanau, caregivers and professionals working with them.
Counties Manukau District Health Board's funder arm is responsible for planning and funding health services, including primary, secondary and tertiary care services, Maori health and Pacific health services, Mental health and services for Older People, for it's resident population.
CMDHB through it's provider arm provides a wide range of specialist secondary services, a selected range of community services, as well as a number of niche specialist tertiary services.
Citizen Advocates are recruited by the Citizen Advocacy office. Staff provide orientation, ongoing support and give assistance to gain further knowledge.
As you get older you may need some support from time to time and a reminder of just how much life you have left in you. Our positive ageing services ensure older New Zealanders have a community-based option to assist them to stay as independent as they want to be in their own homes, and to feel great about it. The Enliven philosophy recognises that some things make for a healthier, happy life, no matter what your age: A sense of community, Friends, family whanau – people you love who love you back, Being able to give and receive, a home of your own choosing, being able to make good decisions for yourself, and most of all, practical support, when, how and where you need it most.
Enliven is staffed by a Service Manager and 5 Service Co-ordinators who are all Health Professionals and oversee 120 Key Support Workers who provide support to clients in their own homes.
Home Based Support Services are provided to clients under contract to Counties Manukau District Health Board Needs Assessment Service Co-ordination, ACC as well as to private paying clients.
